Make a referral about a nurse or midwife using our forms

If you are concerned about whether a nurse or midwife is fit to practise, you can make a referral to us.

Please make sure you have gone through all the previous steps before proceeding to our referral forms below.


Employer form
For employers of the nurse or midwife being referred.

Colleague referral

Third parties

For organisations or individuals making a referral in their professional capacity, where there is no employment relationship with the nurse or midwife.

This may include: other regulatory bodies; a doctor referring a nurse or midwife on behalf of their organisation; or a police officer making a referral on behalf of the police.

Sending your form and evidence

Emails are limited to 25MB each, so please send large attachments in separate emails to ensure they will be received.

You will receive an automatic email response within seven days, confirming receipt and your unique case reference number.

Our staff are available to discuss your referral by telephone, email or letter.

Sharing your referral

  • We may need to share your referral and any associated information with the nurse or midwife concerned, their employer and any other relevant party as required. This helps us to deal with your concerns. We would like your agreement to do this, so we ask that you provide this in the referral form.
  • You can still make a referral without providing your agreement to share the information, but we may still need to act on the information that you have provided and share it as a result. We will explain this to you if this is the case.
  • We may also require your consent for any healthcare provider holding relevant information about your referral (including medical or nursing notes) to disclose that information to us as required for our investigation. We will speak to you about this first if we do.


If you wish to remain anonymous you can still make a referral, however please be advised that you will not receive any updates on the progress of the investigation.

Although the nurse or midwife needs to be aware of the identity of a complainant, we respect patient confidentiality. At hearings, identities are usually kept anonymous. In cases where a complaint of a sensitive nature is made against a nurse or midwife (for example, sexual impropriety) and the patient is the sole witness, we try to make the hearing as easy as possible.

How we'll deal with your information

Initially we may only need to see copies of the documentation supplied as evidence. However, we may require originals if the case progresses to an adjudication committee.

We always hold paperwork securely. When we send information to the nurse or midwife concerned, they are warned that the documentation received is only to be used in connection with the investigation.

We may need to pursue additional information from others, in order to deal with your concerns.
